Patient motivation: the foundation of success

Advice from CooperVision on the soft multifocal contact lens fitting process

Suitable patients may include established, successful distance only contact lens wearers, as well as new wearers who are strongly motivated to remain independent of spectacles.1-3 Motivation is a critical factor for success with soft multifocal contact lenses.1-3

Both pre-presbyopic and presbyopic patients are frequently unfamiliar with the term presbyopia and its implications.2 The need for presbyopic correction is also commonly perceived negatively, often as a sign of ageing.2 Consequently, many patients are aware of visual symptoms for some time before seeking advice from an eye care professional.2

It is important to balance positivity with realism when communicating with presbyopes. Patients should be advised that the aim is to provide functional vision for approximately 80% of daily visual tasks.1,3,4,5 While most activities can be performed comfortably for the majority of the time, patients should also be informed that supplementary reading spectacles may occasionally be required, particularly for fine or sustained near tasks.1,5

Contact lens selection

The principles guiding contact lens material selection, replacement frequency, and wearing schedules are broadly the same for presbyopic and non-presbyopic patients.2 Eye care professionals should prioritise comfort, visual performance, convenience, and minimal disruption to ocular physiology.2

The prevalence of clinically diagnosed dry eye disease and meibomian gland dysfunction increases with age, emphasising the importance of careful material selection to maximise comfort.3

In addition, age-related reductions in corneal endothelial cell density – which are essential for fluid regulation– highlight the need for contact lenses with adequate oxygen transmission to support endothelial integrity.6,7

Initial lens power

Fitting begins with an up to date refraction3 and using the relevant fitting guide to maximise success.3,8

Prescriptions should be configured to maximise positive power in the distance portion, often achieved by loading plus power into the non-dominant eye without compromising binocular distance vision.2

The aim is to prescribe the highest acceptable plus for distance and the minimum plus for near to reduce the risk of over accommodation, particularly in patients with residual accommodative ability.3

To optimise outcomes, trial periods should be conducted using the most accurate prescription on-eye, rather than the closest available option.8

Research shows that patients reporting high overall vision satisfaction at the initial fitting are likely to maintain high satisfaction after one week of wear.8 In addition, patients increasingly expect a fast and efficient fitting experience.

When evaluated via a dispensing study, at a two-week visit, the mean distance and near visual acuity were improved for the clariti® 1 day multifocal with Binocular Progressive System™ when compared with the existing clariti® 1 day multifocal 2 add.‡15
