Actually in an ideal world its should and could be both. There is little at the top end of the scope of practice that is different; MECS, Casualty, Medical Retina Glaucoma all can take please in either assuming appropriate governance in place. Some things done in the HES really ought to be devolved out; casualty triage, post-cats, enhanced glaucoma refinement of referral, stable glaucoma and OHT monitoring. The only big differences are in things like consent listing for surgery, into VEGf injection and laser (like YAG).
